Yes, the insurance is paying for all of this (or at least they say they are). As long as I don't leave northern Utah, they are actually quite good at paying up. If I go out of state, I'm on my own (no coverage). This (combined with zillions of frequent flyer miles) is why I often end up overseas for "big procedures" - if I'm paying in cash anyway, I might as well go to the best.
