For a single or multiple-level ADR? Dr's office despite an authorization by them (now Empire Health Choice) is unsure if they will pay since there's a fine print caveat that the authorized procedure can be up to subsequent review.

Severe, extensive DDD, considered inoperable by Dr. Regan, Lauressen, & some guy at UCLA. Severe foraminal stenosis (guess they can't operate!) and some spinal cord compression that Lauryssen would fix if gets outta hand.

I am in NY and the Empire Plan for university and government employees paid for the operation but not the disk and not the Drs fee. United Health us supposed to pay the Dr but I never filed.

In all, I got about half back.
